Will you call me with lab results?

All patients are expected to return to follow-up on test results. Our cardiologist will discuss results with you in great detail with the aid of diagrams and images. Your questions and concerns will be answered as well. For your safety and privacy results are given in person and not over the phone.

Can I get my prescriptions over the phone?

Our cardiologist prescribes medications in a quantity that would last through your next clinic visit. 4 weeks prior to your running out of the prescriptions, please schedule a follow up clinic visit. At that visit, the doctor will address the ongoing need for your medications or consider dosage readjustment.

How do I prepare for my appointment?

Please arrive 15 minutes early to fill out the forms. You may also save time by printing the forms from the website and filling them out prior to your appointment. If you are unable to keep your appointment, we request that you call us 24 hours in advance to inform us about the cancellation. The appointment would be cancelled if you are late by 20 minutes or more.

How do I obtain a refill on my prescription?

Please call your pharmacy to fax us your prescription request. For your safety and privacy, prescriptions are not given over the phone. Please give us 10 working days to have your prescription refills ordered.

How can I request for my medical records when I am out of town?

Please download the authorization to receive medical record form from the new patient form list. Please fill it out and fax it to 480-361-9969. Please allow 10 business days for completion of request.

How do I schedule an appointment?

In order to schedule an appointment the following is required:

  • Your full name and date of birth
  • Best phone number to contact you
  • Name of your insurance as displayed on your insurance card
  • Your insurance subscriber ID (displayed on the front of the card)
  • Contact number for your insurance (located on the back of your card)

What do I need to bring to my appointment?

Please bring:

  • Your insurance card and drivers license
  • For HMO plans, referral from your doctor
  • Your current medication list
  • Any other records that you may have
